August School Health Portal

How to Access August Schools
1. Log in to your Blackbaud account.
2. Click the August Schools link under the Resources tab.
3. Go to the Requests tile to view and complete all required health forms.
For Student-Athletes (Grades 5 and Up)
1. A Preparticipation Physical Evaluation Form is required before the start of the sports season, along with the Sudden Cardiac Arrest and Concussion form.
* The physical is valid for one year from the date of the appointment and must be signed by a licensed physician.
2. Once completed, the form must be uploaded to August Schools.
