END OF YEAR MEDICATION PICK UP (MUST PICK UP BY MAY 23)

Dear Parents and Guardians,

My name is Amy Chesna, and I am so happy to be the new School Nurse at FSA.

End of the year check off list:

It is that time of year, to be working into your day, picking up unused medication from the Clinic. This includes, Over-The-Counter, Prescription, Inhalers, and Epi Pens.

These medications will NOT be able to be held over the summer. Please be sure to stop in before the last day of school, May 23rd, before 3:30 pm. ALL medication not picked up from the clinic, before the last day of school, will be considered abandoned. Abandoned medication will be destroyed in accordance with local, state, and federal guidelines, by the nurse and an administrator. We do not store medication over the summer to use for the next year.

Please be on the lookout for our NEW School Nurse webpage in the FSA Website, under School Life. I am excited to get this up and running early summer. In this site, you will be able to download all the forms that are needed to get filled out over the summer, that need physicians signatures, for the next school year. These forms will include Medication forms, (Other-the-Counter & Prescription), and all the Health Care Plans pertaining to your child’s Medial Health Conditions. New Care Plans MUST be filled out and signed by the doctor each school year. We do not keep care plans from the previous year. ALL medications, and ALL care plans can be brought to the School Nurse at Orientation, August 2nd, 2019.

Lastly, make sure your child is up to date on their immunizations. Any time your child receives a vaccination, please send in an updated Georgia Registry of Immunization Form #3231 to school. We are required by state to keep updated vaccine records for all of our students. Please refer to the chart below to make sure your child has all the required immunization.
