FSA will now utilize Magnus Health to store student physical forms required for athletics.

Physical forms must be completed by a physician.

Only students who are deemed medically eligible will be allowed to participate in athletic teams.

Physical forms are good for one calendar year.

Students may not participate in any FSA athletics without a valid physical form on file.
